Wellopp Connect works closely with providers, payers, hospitals, and pharmacies to develop a customized, local network of community resources focused on improving patients’ social determinants of health (SDOH) factors. This network extends much-needed support to help minimize barriers to care, reduce preventable readmissions, improve health outcomes, and lower downstream healthcare costs.
